Notice: We are writing to inform you that BH ACO, LLC will no longer be participating in the ACO REACH Model with the Centers for Medicare & Medicaid Services (CMS), effective March 31th, 2026. As required by CMS, we must notify of changes related only to ACO-specific Benefit Enhancements (BE). Furthermore, any services furnished under the Benefit Enhancement will no longer be covered by Medicare and the Beneficiary may be responsible for the payment of such services. The Benefit Enhancements elected by our ACO for PY2026 include: ​

• Care Management Home Visit ​
• Home Health Homebound Waiver ​
• Post Discharge Home Visit ​
• Skilled Nursing Facility (SNF) 3-Day Stay Waiver ​

Services furnished under Benefit Enhancements will no longer be covered by Medicare and the Beneficiary may be responsible for the payment of such services as of June 29, 2026. There are no Beneficiary Engagement Incentives elected by our ACO for PY2026. ​
